  • All visits and services will be direct pay from patients
  • If your insurance has out of network benefits you may send a claim to them for any reimbursement
  • Patients with Medicare/Medicaid are not able to file a claim for reimbursement

Policies

Santa Rosa Women’s Center strives to provide our patients with quality care delivered in a timely and efficient manner. We encourage you to read the policies below so you will be prepared for your office visit.

If this is your first visit, please arrive approximately 30 minutes prior to your appointment time in order to complete paperwork and get checked in. For future visits please arrive 10-15 minutes prior to your appointment. We ask everyone who will be late for an appointment to please call. If you are more than 15 minutes late, we may have to reschedule your appointment.

If you cannot keep your scheduled appointment, please call the office in advance to cancel or reschedule. You may be charged a $50 no show fee if you fail to notify the office.

Please be prepared to pay any deductible or copay that you are responsible for when you come for your appointment. Payment is expected at time of service. Payments must be made with a credit/debit card or cash. We do not accept checks.

We encourage our patients to be aware of any refills on medications that you need. Our office policy is to refill all medications by appointment only.

Insurance

Please bring your insurance card with you for each visit. It is your responsibility to update our office with any changes in your insurance on each visit. We do participate in many insurances plan, but you should check with your insurance prior to being seen to verify that Santa Rosa Women’s Center is in network with your insurance. You will also need to inform our office if your insurance requires you to have your lab work, specimens or outpatient procedures performed by a specific lab/facility.

If you do not have your insurance card, or we are unable to verify your benefits/eligibility, you will be expected to pay in full at the time of service.
